Salford’s Centre of Contact is a team of customer care and health and social care professionals who can offer advice, information and support to the public and professionals on a range of health and social care topics.
It provides a single point-of-contact for intermediate care, adult health and social care, and district nursing services. These are services that support people to live at home. They can help people stop needing to go to hospital or residential care.
The team are now recruiting to various positions within the team. These positions will be integral to the work of Salford Together who, along with partners, are working together to provide more integrated care and new models of care to meet the needs of our changing population.

2.5 x Community Nursing Coordinator
The role of the Community Nursing Coordinator will enhance the current community nursing provision at the Centre of Contact.
The Community Nursing Coordinator will build on the vision of the centre of contact, providing a multi-disciplinary approach to holistic support and prioritisation at the first point of contact for services users, providing triage, signposting and navigation, onward referrals, telephone assessments, and general clinical support where required.
The role is required to meet the clinical needs of service users phoning into the single entry point, Community Nursing and Intermediate Care Services based within the Centre of Contact. The post holder will work with relevant colleagues to ensure existing pathways are appropriate and effective. The post holder should enjoy team work and be keen to develop their clinical assessment skills.

As a key member of the PMO, supporting the senior leadership team to meet its objectives, you will have unique opportunities in joining a leading-edge organisation at the start of the next phase of its transformation journey.
