We would like to invite you to take part in a short online survey, which aims to understand your experience of integrated care and transformation.  Findings will help us to know where we are doing well and where we need to do better and will be used to inform future transformation work.

Who is this survey for?

It is for Salford staff and organisations working in:

  • Community health and social care, including mental health and also voluntary, community and social enterprise
  • Integrated Care
  • Primary Care
  • Health and care commissioning.

The survey is intended both for people working directly with our citizens, or those who support in another way.  This survey will be completed by colleagues working throughout the health and social care workforce across Greater Manchester, so it will also help us know how Salford is doing alongside other areas across GM.

 

Some Background:

It might be helpful for you to understand some of the background and terminology before you complete the survey:

  • Salford’s Locality Plan describes Salford’s shared partnership system vision for improved health and wellbeing for people in Salford.
  • This plan links our understanding of needs with our coordinated response to them.
  • Working differently through new models of integrated care and transformation is a key element of our planned response.
  • Across Greater Manchester and Salford these integrated models have meant some changes and we would like to know what you think about these changes and how they are affecting you.

 Examples of the changes have included working together to:

  • Transform and integrate health and care services
  • Establish a Local Care Organisation to organise the delivery of care (known as the ICO – Integrated Care Organisation in Salford)
  • Improve the way we commission services collaboratively (known as Integrated or joint commissioning in Salford).
