The Care Home and Supported Living Multidisciplinary Group is focused on the ‘needs a lot of help’ level, to provide those who require a high level of support from health and social care services or those in care homes receive personalised and high quality care, no matter where they are.
Prior to admission to a nursing or residential care home, a comprehensive assessment of their needs will be undertaken, considering alternatives to residential care, for example whether the person could be supported to stay in their own home.
An individualised care plan will also be developed by the care coordinator, and this will be reviewed regularly, or when the person’s needs change.
For those who are nearing the end of their life, the person’s wishes, needs and care preferences will be agreed, documented, shared and understood by all of the services involved in the person’s care, to ensure they are supported to die in their preferred place.
