This free event is about Salford’s journey to an integrated care system. This learning experience will showcase how we’ve come together as a partnership to improve system outcomes. Places are limited so don’t miss out on this unique learning opportunity.
What is Salford Together?
Presenting Salford’s Integrated care system – our Integrated Care Organisation (bringing together acute and community physical and mental health services and social care), Salford Primary Care Together (GP provider organisation) and Integrated Commissioning. We will cover how we established our system including, Organisational Development, our Locality Plan and Transformation plans for neighbourhood population health.
What will I learn?
Introducing the history of the Salford partnership and how we came together. We will discuss our Integrated Care programme for older adults and joint working initiatives. We will also explore the vital importance of system leadership and vision, showcasing how we have developed the Integrated Care System.
We will cover the full business case for development of the ICO and GP provider organisation covering key aspects such as Health & Social Care Joint Commissioning pooled budget, Due Diligence, contracting, finance, legal, HR, Organisational Development, Governance, IM&T.
